This is a 46-year-old woman found to have the PALB2 genetic mutation, increasing her risk for breast cancer. She underwent a bilateral prophylactic nipple-sparing mastectomy with immediate DIEP flap breast reconstruction. She then underwent a reconstruction revision surgery with fat grafting.
