This is a 66-year-old woman found to have right invasive ductal carcinoma. She underwent a bilateral nipple-sparing mastectomy with immediate DIEP flap reconstruction. The patient desired more volume and projection, so three months later she completed her reconstruction with a revision surgery in which she had fat grafting and bilateral implants placed behind the living tissue.
